At Apex Support Services, our team, including the Program Director, QMHP In-Home Counselor, or Clinical Director, conducts thorough assessments for each client before program admission. From these assessments, personalized support plans are crafted to meet individual needs.
Our services encompass assessing clients' needs, setting personalized goals, developing support plans, implementing interventions, and monitoring progress. It's a collaborative process involving professionals and clients, aiming to address individual needs effectively and empower clients on their journey to mental wellness.
We host organized sessions where individual speakers address youth and families on various topics such as street and gang violence, substance abuse, and other relevant issues. These sessions provide a platform for education, discussion, and support, aiming to enhance awareness, coping skills, and overall well-being within the community.
We promote addressing specific items identified in the support plan, with a focus on social skills development and the implementation of new coping skills and behavioral intervention strategies. These sessions provide personalized guidance and support to help individuals navigate challenges, develop resilience, and achieve their therapeutic goals outlined in their support plan.
We specialize in developing positive communication skills, addressing family issues impacting family dynamics, and establishing clear rules and consequences to promote family resilience and self-esteem. Our team provides services aimed at restructuring family dynamics and implementing effective parenting skills tailored to each family's needs, fostering a nurturing and supportive environment for growth and healing.
Our Anger Management program is designed to empower individuals with comprehensive skills to manage anger effectively. Participants engage in an eight-week program consisting of five-hour weekly sessions. These sessions encompass various activities such as role-playing, self-advocacy, and self-determination exercises. Our group sessions, limited to eight participants, are facilitated by two experienced counselors and meet for two hours each week. Additionally, participants receive three hours of one-on-one support per week, including family involvement as needed. This personalized approach allows for the development of self-control, stress management, and effective communication strategies. Transportation to group sessions is provided, along with initial assessments to identify specific challenges in community, home, and school environments.
The interaction with school personnel includes scheduled visits to the school guidance counselor or responding promptly to any calls from the counselor to provide necessary updates or information. We prioritize open communication and collaboration with school staff to ensure comprehensive support and continuity of care for our clients.
We provide 24-hour crisis intervention services, responding promptly to calls from both families and individuals to offer support and help stabilize crisis situations. Additionally, we assist with scheduling medical appointments and provide transportation when needed.
We facilitate social skill development through interactive role-playing exercises and therapeutic recreational activities. These activities promote appropriate social interaction and enhance communication strategies, helping individuals build confidence and resilience in social settings. Additionally, we host family nights at least once a week to further enhance bonding and communication within our community.
We offer a comprehensive program tailored to individuals aged 15 to 18, aimed at fostering self-sufficiency and life skills necessary for successful integration into society. Throughout the 20-week program, participants engage in a multifaceted curriculum covering various aspects of independent living. This includes personalized coaching in budgeting and financial management, hands-on sessions tailored to meal planning and preparation, guidance on navigating the job market with assistance in resume building and interview techniques, as well as training in housekeeping skills for maintaining a clean and organized living space. The program structure includes a combination of one-on-one counseling sessions conducted in the comfort of their homes and interactive group activities. These group sessions provide opportunities for participants to collaborate on projects, embark on educational tours to explore potential career paths, and visit institutions such as trade schools, college campuses, financial institutions, and public libraries. Through this immersive and hands-on approach, we empower young individuals with the knowledge, skills, and confidence needed to navigate the challenges of adulthood successfully.
